The NIH is in the process of changing the forms they require in order to move to the electronic submission of grants. This is being done in a phased process. Samples of the new SF424 forms are below. The SRSP can help you determine the appropriate forms to use and help you complete each of them.
NIH has released new PHS398 forms for paper submission and new SF424 forms for electronic submission.
If your NIH Grant exceeds $500,000 in direct costs in any one year, written permission must be gained from the NIH Program Officer before the grant is submitted.
If your proposal uses any form of Human Subjects or Human Cell Lines you must let the SRSP know as soon as possible to ensure that your application is submitted with the appropriate forms and assurances.
